  • Parents and guardians should monitor their children for symptoms of infectious illness every day before they leave for school through home-based symptom screening.


    As with any illness, it’s encouraged to stay home when you are sick. Students and staff with symptoms of infectious illness including COVID-19, influenza, and respiratory syncytial virus (RSV) should not attend school or work and must be fever-free for 24 hours without fever-reducing medication prior to returning.


  • Kindergarten Registration Medical Requirements

    All incoming kindergarten students are required to have the following state required examinations: physical, dental, vision (completed by an optometrist or ophthalmologist, not your family physician).

    • You may start the exam process now (winter/spring 2023) in order to obtain these examinations in time for your kindergartener beginning school in August 2023.
    • If your child will be needing any of the following items, please contact your school nurse prior to your child’s medical exam so that appropriate forms may be completed by parents & physicians.
    • Some examples are listed below:
      • Medication needed during school hours
      • Diet prescription
      • Asthma action plan
      • Food allergy
      • Seizure action plan
      • Diabetes plan
    • An original birth certificate & immunization record needs to be turned in to the nurse at the time of registration.



  • Over-The-Counter and Prescription Medications

    • All medications (both OTC and prescribed) must be brought to school by a parent/guardian. Students are not allowed to carry or possess medications on their person or in their locker. Violations of this rule will be considered violations of Policy 5530 and discipline will apply.
    • All medications must be in the original container with proper labeling.
    • A new Authorization for Medication and Treatment form must be completed and signed each year by the parent/guardian for any OTC medications (acetaminophen, ibuprofen, TUMS, etc.).
    • A new Physician Authorization Form must be completed in addition to the Authorization for Medication and Treatment form for any prescription medications. 
    • For emergency medications such as Epi-Pens or asthma rescue inhalers, a Physician Authorization Form must be completed and signed by the physician in addition to the forms mentioned above.
    • It is recommended that, if at all possible, daily OTC and prescribed medications be scheduled either before or after the school day.
