Health Forms

CHIRP stands for the Children and Hoosiers Immunization Registry Program. It is a secure, web-based database administered by the Indiana Department of Health used to permanently store electronic immunization records for people of all ages. The registry allows healthcare providers, pharmacies, and schools to securely track and verify vaccination histories. The primary purposes of CHIRP include:

Preventing Revaccination: Helps doctors view a patient's complete vaccination history across different providers to avoid unnecessary, duplicate shots.
Tracking Requirements: Notifies providers and school nurses of which vaccinations are due or required.
Record Access: Allows individuals and parents to access, download, or print their official vaccine records for school, travel, or employment.

CHIRP is optional for parents and guardians, so HCSC is required to get parent/guardian consent. This form is not required to be completed each year. If a CHIRP form is not on file for your student, a school nurse will contact you.

Medication Forms

A Physician Authorization form is a required form when your student needs any prescription medication during school hours. This includes emergency medications. The medications must be in their original prescription bottle or packaging when they are turned into your school nurse. The form must be signed by a physician and parent/guardian and turned into your school nurse. A new form must be completed each school year. Physician Authorization Form

An Authorization for Medication or Treatment form is a required form when your student needs any over-the-counter medications during school hours. This includes Tylenol, Advil, TUMS, or allergy medications, such as Benadryl or Zyrtec. The form must be signed by a parent/guardian and turned into your school nurse. The medication must be in the original packaging and sealed when it is turned in for your student. A new form must be completed each school year. Parents and guardians should monitor their children for symptoms of infectious illness every day before they leave for school through home-based symptom screening.

As with any illness, it’s encouraged to stay home when you are sick. Students and staff with symptoms of infectious illness including COVID-19, influenza, and respiratory syncytial virus (RSV) should not attend school or work and must be fever-free for 24 hours without fever-reducing medication prior to returning.

New or Transferring Students

Welcome to Hanover! The following medical information is needed for your student to attend. Please call your school nurse or the Director of Health Services (219-374-3533 OR kdonaghue@hanover.k12.in.us) if you have any questions. Each school nurse with their contact information can be found underneath this information.

1. Immunization Records

2. School Physical. An IHSAA Sports Physical is needed for Athletics only.

3. Medication Forms for both OTC and prescribed medications.

4. Medical Forms for Anaphylaxis & Food Allergy, Asthma, Diabetes, and Seizures.

5. Please notify the School Nurse of any other medical issues that may affect your student's time during school hours. Please notify the Athletic Director (219-374-3806 OR rbridges@hanover.k12.in.us) of any medical issues that may affect your student's time during athletics.

Vaccination Compliance

Attention Parents and Guardians,

THE DEADLINE TO BE IN VACCINATION COMPLIANCE IS FRIDAY, SEPTEMBER 11TH, 2026.

YOUR CHILD MAY NOT COME TO SCHOOL ON MONDAY, SEPTEMBER 14TH, 2026, IF WE DO NOT RECEIVE PROOF OF IMMUNIZATIONS.

If you are unable to get the required vaccine(s) by September 11th, 2026, you need to contact your school nurse ASAP with the date your child will be receiving the vaccine(s). With this notice, you will find the Indiana 2026-2027 Required and Recommended School Immunizations and information from the Lake County Health Department Immunization Clinic. Please take note of the required vaccinations and grade level for your student to ensure they are compliant.

If you have any questions, please reach out to your school nurse.

This is a state requirement in order to attend school. Students must have their completed required immunizations, or a religious/medical exemption form on file with the school nurse.

If you choose to decline, the Immunization Objection Form can be found on the Hanover website under the Health Services Department under Health Forms and a copy has been attached to this notice.

Over-The-Counter Prescription Medications

All medications (both OTC and prescribed) must be brought to school by a parent/guardian. Students are not allowed to carry or possess medications on their person or in their locker. Violations of this rule will be considered violations of Policy 5530 and discipline will apply.
All medications must be in the original container with proper labeling.
A new Authorization for Medication and Treatment form must be completed and signed each year by the parent/guardian for any OTC medications (acetaminophen, ibuprofen, TUMS, etc.).
A new Physician Authorization Form must be completed in addition to the Authorization for Medication and Treatment form for any prescription medications.
For emergency medications such as Epi-Pens or asthma rescue inhalers, a Physician Authorization Form must be completed and signed by the physician in addition to the forms mentioned above.
It is recommended that, if at all possible, daily OTC and prescribed medications be scheduled either before or after the school day.

Kindergarten Registration Medical Requirements

All incoming kindergarten students are required to have the following state required examinations: physical, dental, vision (completed by an optometrist or ophthalmologist, not your family physician).

You may start the exam process now in order to obtain these examinations in time for your kindergartener beginning school in August 2026.
If your child will be needing any of the following items, please contact your school nurse prior to your child’s medical exam so that appropriate forms may be completed by parents & physicians.
Some examples are listed below:

- Asthma action plan
- Food allergy
- Seizure action plan
- Diabetes plan
An original birth certificate & immunization record needs to be turned in to the nurse at the time of registration.
