Attention Parents and Guardians,

THE DEADLINE TO BE IN VACCINATION COMPLIANCE IS FRIDAY, SEPTEMBER 13. 

YOUR CHILD MAY NOT COME TO SCHOOL ON MONDAY, SEPTEMBER 16, IF WE DO NOT RECEIVE PROOF OF IMMUNIZATIONS. 

If you are unable to get the required vaccine(s) by September 13th, you need to contact your school nurse ASAP with the date your child will be receiving the vaccine(s). With this notice, you will find the Indiana 2024-2025 Required and Recommended School Immunizations and information from the Lake County Health Department Immunization Clinic. Please take note of the required vaccinations and grade level for your student to ensure they are compliant.

This is a state requirement in order to attend school. Students must have their completed required immunizations, or a religious/medical exemption form on file with the school nurse. 

If you choose to decline, the Immunization Objection Form can be found on the Hanover website under the Health Services Department under Health Forms and a copy has been attached to this notice.

Immunization Requirements 

Immunization Form

All incoming kindergarten students are required to have the following state required examinations: physical, dental, vision (completed by an optometrist or ophthalmologist, not your family physician).

 

• You may start the exam process now (winter/spring 2023) in order to obtain these examinations in time for your kindergartener beginning school in August 2023.
• If your child will be needing any of the following items, please contact your school nurse prior to your child’s medical exam so that appropriate forms may be completed by parents & physicians.
• Some examples are listed below:
  • Medication needed during school hours
  • Diet prescription
  • Asthma action plan
  • Food allergy
  • Seizure action plan
  • Diabetes plan
• An original birth certificate & immunization record needs to be turned in to the nurse at the time of registration.

• All medications (both OTC and prescribed) must be brought to school by a parent/guardian. Students are not allowed to carry or possess medications on their person or in their locker. Violations of this rule will be considered violations of Policy 5530 and discipline will apply.
• All medications must be in the original container with proper labeling.
• A new Authorization for Medication and Treatment form must be completed and signed each year by the parent/guardian for any OTC medications (acetaminophen, ibuprofen, TUMS, etc.).
• A new Physician Authorization Form must be completed in addition to the Authorization for Medication and Treatment form for any prescription medications. 
• For emergency medications such as Epi-Pens or asthma rescue inhalers, a Physician Authorization Form must be completed and signed by the physician in addition to the forms mentioned above.
• It is recommended that, if at all possible, daily OTC and prescribed medications be scheduled either before or after the school day.
